Etsy Sellers: What contact info do you include in your packaging?

Do you include the address of your Etsy shop? Blog? Email? Name? Both my etsy shop, my blog and my email are my name. I wasn't sure which to include.

Ex: JaneDoe
JaneDoe.etsy.com
JaneDoe@gmail.com

Do sellers normally include an email contact with their package? Any advice is appreciated, Thanks!
