I am not one of those people who is able to plan outfits in advance - I end up standing in front of my closet, running 5 minutes late, tapping my foot and wondering why I don't have anything to wear!

It depends. Sometimes I'll have an idea of what I want to wear, but usually I'll just wakeup and throw on whatever speaks to me, haha. I have a friend who plans out her outfits for the week on Sunday night. For some reason this really annoys me.
I am a humongous nerd with OCD, so I plan all my outfits the Sunday before. Otherwise, I'd never get to work on time. I also keep a log of everything I wear during the workweek

I try to. I always used to think I had nothing to wear, but I found that the problem is that some outfits just required more thought than I was willing to give at 7:30am in the morning so I was always going with the easy choice. Also, I had trouble remembering when the last time I wore an outfit was. So, I started an Excel spreadsheet where I put down what I wear every day. It helps me to "remember" outfits and also to keep track so I am not wearing the same things all the time. Sometimes I'll forget for a whole week though. I also lay out my clothes the night before so I don't just grab the easiest outfit all the time. If something is laid out for me, I will wear it.
Sometimes. It depends. And by planning, I mean, I'll be in bed, almost ready to go to sleep when I'll get this brilliant idea of a new way to pair this top with these pants or layer this cami with that top. The only time I really, really plan ahead is for a special event, but usually, I just have to wear whatever 'feels' right at the time (this makes packing for a trip sooooo difficult for me--I constantly overpack. Who knows, maybe I'll feel like wearing that red cardigan that's been at the back of my closet for the past year).
I do, however, keep a log of outfits that I have worn that I think are just fabulous. I'll put them on one of my mannequins and snap a photo, so that I can remember it. But, that doesn't mean I'll wear it again, in the exact same way. I really don't think I've ever worn the exact same outfit twice (and by that I mean, I am just constantly trying different combinations, colors and accessories).
usually the night before as i'm drifting off to sleep i'll try and get an idea of what i want to wear the next day. i'm not a morning person so i have to have a general idea of what i want to wear or i'll make myself late. when this happens i also end up wearing something that i don't like that much (or all black) b/c *had* to leave the house.
I try really really hard to plan my outfits the night before. Or sometimes when I'm in the shower during the morning, I'll be talking to myself about what shirt might work with the new skirt I have or whatever.
However, I do have a few go-to outfits that I wear in the event that I feel horrible that day.
i end up pulling things together as i get ready for the day. tonight was the first night in a LONG time that i actually pulled stuff together, but only b/c i found some cool stuff thrifting last week. most of the time, it depends on my mood, the weather & whether or not i shaved my legs.
Our weather is too unpredictable here, and I hate when my outfit isn't weather appropriate. I usually think about what I'll wear when I am out walking my dog in the morning.
Sometimes I have a general idea that I want to incorporate something into my week. For example, I know that I want to wear my pink cashmere sweater some time this week, but I don't know when yet.
I have a serious case of OCD...and yes, it extends to my wardrobe. Rather than planning out what to wear each day...I have the hangers that hold the whole outfit...and every piece of clothing I own goes on them...into an outfit. So...each day, I can just grab the next hangerful as I head off to the shower.
I need to, I'm always running late in the morning. I had the best intentions. Since I moved, I now have a dressing room and bought myself a clothing rack. For awhile I would put all my outfits for the week on it. Now all my clothes end up on it. I need to do better again just to make my mornings easier.

I've thought about doing this before, but what do you do with shirts that can be worn with several different bottoms or vice versa?
I typically do the night before. My closet is so jammed pack with clothes and I have so many accessories that I could never do it in the morning. Primarily b/c I am not a morning person.
I plan my outfits on Sunday night, as a matter of fact I just finished.
I chose 5 outfits, right down to the jewelry and shoes and sometimes bag, hang them on a portable rack. Each morning I pick whatever is appropriate for the weather or my mood, occasionally I'll make a minor switch but 95% of the time I wear what I've selected.
This saves me so much time and stress in the mornings.
